29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees. Shore side Public Works Building, Carpenters Shop, main floor area: On or about 26 June 2012 and times prior thereto, the Woodtek dust vacuum electrical plug was damaged. This exposed employees to a 120 volt electrical shock hazard.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective. Shore side Public Works Building, Electrical Shop: On or about 26 June 2012 and times prior thereto, a 120 volt, electrical outlet in the main shop area, north wall, tested no ground with a sure tester 61-164 tester. This exposed employees to an electrical shock hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws. Shore side Public Works Building, Electrical Shop: On or about 26 June 2012 and times prior thereto, a 120 volt, 25 foot, blue extension cord had its plug conductors exposed due to lack of strain relief. This exposed employees to an electrical shock hazard.

This exposed employees to a 120 volt electrical shock hazard.
