29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    (a) Jobsite, 5419 Forbes Avenue, Pittsburgh, PA:  On or about 6/27/12, employees engaged in residential construction activities approximately 16-20 feet above the ground were not protected from falling by the use of guardrail systems, safety nets or personal fall arrest systems.  In addition, the employer did not demonstrate that it was infeasible to utilize conventional fall protection systems and did not develop any alternative fall protection plan.

An accident prevention program should include, but are not limited to: methods for inspection of tools and equipment in use at worksites, protective equipment necessary for employee protection and methods for training employees on hazards associated with the type of work being performed.
