29 CFR 1910.23(d)(1)(iv): The employer did not ensure that stairs with four or more risers that were more than 44 inches wide but less than 88 inches wide had a stair railing on each open side and a handrail on each enclosed side. (a) North side of terminal - Employees used a flight of seven stairs to enter the terminal from the employee parking lot and leave the terminal to go to the employee parking lot each day. The stairs were 48 inches wide and did not have a handrail installed at the enclosed side, thereby exposing employees to the risk of falling down the stairs on or about July 3, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

(a) Reading Terminal - The employer did not follow the prescribed formatting for the workplace hazard assessment. The identity of the workplace evaluated was not included, the identity of the person certifying the evaluation was not included and the document was not identified as a "Certification of Hazard Assessment" on or about July 3, 2012.
