29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder���s length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a) Location - Riverside Drive, Methuen, MA:  On or about 7/5/12, employees were not protected from falling while using a ladder that did not extend at least three feet above the upper landing surface.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502.    (a) Location - Riverside Drive, Methuen, MA:  On or about 7/5/12, employees were not protected from falling while working on a roof greater than six feet above a lower level.    Christopher Evans D/B/A Evans Exteriors was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926. 501 (b) (13) which was contained in OSHA Inspection Number 314418336, Citation Number 01, Item Number 2 a Issued 4/7/10 with respect to workplace located at 945 Riverside Drive, Methuen, MA, 01844. The final order date for the citation was on 4/12/2010.
