29 CFR 1910.23(e)(1):     A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail shall be smooth-surfaced throughout the length of the railing. The intermediate rail shall be approximately halfway between the top rail and the floor, platform, runway, or ramp. The ends of the rails shall not overhang the terminal posts except where such overhang does not constitute a projection hazard.    a) At the time of inspection, in the tire stock area, the most interior walkway does not have horizontal railings at the correct heights exposing employees to fall hazards.    b) At the time of inspection, in the tire stock area, the center walkway does not have horizontal railings at the correct heights exposing employees to fall hazards.    c) At the time of inspection, in the tire stock area, the exterior walkway does not have horizontal railings at the correct heights exposing employees to fall hazards.

29 CFR 1910.147(c)(6)(i):     The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.      a) At the Monro Muffler, Brake and Service shop, located on South Broadway in New Philadelphia, Ohio, the employer did not conduct an annual or more frequent inspection of the energy control procedures by each authorized personnel to ensure that they were properly implemented during the repair and service work on the following equipment but not limited to: Air Compressor, Automotive Lifts, Oxygen Generators, Air Filters, Tire Changer, and Air Condition Refrigerant Recovery Machine.

29 CFR 1910.305(g)(1)(iv)(A):     Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:      As a substitute for the fixed wiring of a structure;    a) At the time of inspection, at the alignment machine station, an approximately 10 foot black extension cord servicing an alignment machine computer and printer, was used as a substitute for fixed wiring exposing employees to a fire hazard.
