29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. Employees using nail guns at the jobsite located at 1013 N. Coolidge, Wichita, KS 67203 were exposed to struck-by eye hazards in that eye protection was not worn.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE. Employees replacing the roof of the house located at 1013 N. Coolidge, Wichita, KS 67203 were exposed to fall hazards of approximately 10 feet eave to ground in that, fall protection was not used by employees. The roof pitch was approximately 6 in 12.
