29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper surface to which the ladder was used to gain access. (a) On or about 07/06/2012 at 131 Harrow Road, Westfield, NJ 07090: The ladder provided did not extend 3 feet above the working surface of the roof to allow for safe access on and off the roof. Jireh Home Improvement LLC., was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.1053(b)(1) which was contained in OSHA Inspection Number 314978925, Citation #1, Item #1, issued on 08/11/2011 with respect to a work place located at 500 Town Green Drive, Elmsford, NY 10523 and became a final order on 08/30/2011.
