29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a) On or about 7/5/2012 in the afternoon, Pedestrian Bridge: Employees performing decking work 33 feet above the lower elevation were not utilizing any fall protection.

29 CFR 1926.760(d)(1): Guardrail systems, safety net systems, personal fall arrest systems, positioning device systems and their components did not meet the criteria set forth in 1926.502: a) On or about 7/5/2012 in the morning, Pedestrian Bridge: Employees performing decking work 33 feet above the lower elevation utilized a horizontal life line system that allowed for over 12 feet of free fall distance and was not padded where it was in contact with structural steel edges.
