29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: On or about Wednesday June 13, 2012 in building 29702 on the first floor; The employees working on the lower level performing masonry work are exposed to a trip and fall hazard from the unprotected floor opening, which was approximately 33 inches in depth, 19 inches wide and 19 feet long on both sides of the concrete wall being built by the employees.
