29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident: At the wash rack area in the yard where employees power wash drilling mud from tanks; the electric disconnects for the various equipment are all mounted on one wall behind the employee change house and were not identified such as but not limited to the following: a. The Eaton Cutler-Hammer, Heavy Duty Safety Switch, 30A, 600V-, 60Hz, 250V, for the Pressure Washer Disconnect. b. The Square D, Heavy Duty Safety Switch, 600Vac, 60A, for the Mud Plant Dirty Water pump Disconnect. c. The Square D, Heavy Duty Safety Switch, 600Vac, 60A, for the Mud Plant Fresh Water Pump Disconnect. d. The large Safety Switch disconnect for the Water Well.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employees ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: At the wash rack, employees used a high pressure power washer to clean drilling mud from the inside of tanks and were provided 3M half-masked respirators to wear without having a medical evaluation performed prior to their use.
