29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:       a) Residential jobsite, various areas of the roof: On July 5, 2012, employees performing roofing activities which included applying tar paper on a 8 in 12 pitched roof, did not properly utilize the personal fall arrest system that was in place.  Employees wore harnesses but were not attached to the lanyard.      Note) Abatement certification is NOT required for this item.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:     a) Rake edge of residential jobsite: On July 5, 2012 employees used an extension ladder that did not extend three feet above the area where they were using it for access to the roof.  The ladder was positoned at the lower area of the rake edge of an 8 in 12 pitched roof.    Note) Abatement certification IS required for this item.
