29 CFR 1926.502(d)(16)(iii): Fall arrest systems when stopping a fall were not rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level: (a) Jobsite: Bakersville-Edie Road Bridge, Somerset, PA 15501; On or before July 6, 2012, employees working at approximate heights between 6-15 feet were not protected against falling to ground level in that the protection provided would allow a fall distance of approximately 18 and 1/2 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Jobsite: Bakersville-Edie Road Bridge, Somerset, PA 15501; On or before July 6, 2012 employees were not trained in the proper clearance necessary for using a fall protection system in that a 6 foot shock absorbing lanyard was connected below 18 and 1/2 feet.
