29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Worksite- employees were observed working at the edge of a roof approximately 12' in height. No fall protection was provided; on or about 06/29/12.    Note:  In addition to abatement cerification, the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.    Foreview Development was previously cited for a violation of this Occupational Safety and Health Standard, Its equivalent, or essentially similar standard 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 315669754 , citation number 1, item number 4, issued on 10/19/2011 and became final order on 01/25/12.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) Worksite- Employees were not wearing protective helmets and were working directly below and next to the moving arm of a telescoping material handler; on or about 06/29/12.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
