29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph: a) Basement, East Wall - A Square D, 220/110 volts, main breaker load center, was found with the front panel cover removed. Construction workers were exposed to energized conductors, on or about June 19, 2012.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) Basement - A dual receptical box, designed exclusively for wall mounting, was laying on the floor and used for temporary power. Employees were exposed to electrical shock hazards on or about June 19, 2012. b) Basement - Romex cable, designed for permanent installation, was used as temporary wiring. The cable was used to provide electrical power to the dual receptical box. Employees were exposed to electrical shock hazards on or about June 19, 2012.
