29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive siderails:    (a) On or about July 6, 2012, and at times prior, employees were not protected from electrocution and fall hazards in that they used an aluminum ladder to access the roof right next to the point where the building's electrical wires entered the building. 29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    (a)  On or about July 6, 2012, and at times prior, employees were not protected from fall of greater than 15 feet while reroofing a building.    Miller Roofing was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent, which was contained in OSHA inspection 313720211, Citation 1, Item 3a, issued June 25, 2010, with respect to a workplace located at 2800-02 Greenwood, Pueblo, CO, 81002.  This citation became final order on July 27, 2010.      Miller Roofing was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent, which was contained in OSHA inspection 310596051, Citation 1, Item 4, issued March 27, 2008, with respect to a workplace located at 1212 Cedar, Pueblo CO, 81004.  This citation became final order on April 14, 2008.     Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
