29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: On or about July 1, 2012, employee(s) and subcontractor were exposed to electrocution, electric shock, and arc flash explosion hazards due to no personal protective equipment being worn while preparing substations 3A and 3B, feeders A-6 for cleaning. The substations were rated at 1,200 amps and 13,800 volts. The following actions were taken without any personal protective equipment being worn: (a) using a hot stick and voltage tester inside the cabinets of substations 3A and 3B, feeder A - 6. (b) applying safety grounds to substations 3A and 3B, feeder A - 6
