29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support: (a) On or about June 28, 2012, employees were exposed to a potential fall hazard of approximately 10-12 feet while using an improperly installed ladder where the side rails did not extend at least 3 feet above the roof surface they were accessing.
