29 CFR 1910.217(b)(7)(v)(a): Each two-hand control for single stroke operations on mechanical power press(es) using part revolution clutches was not arranged by design, construction and/or separation so that the concurrent use of both hands was required: a) On or about 07/09/12, in the production areas, a Roussell Punch Press (Model 4SS44, Serial EKSAS11364, 44 ton) two-hands control did not require concurrent use of both hands to initiate a stroke of the press. In addition, the two-hands control buttons were close enough to each other that they could be operated by one hand/arm. The distance between two-hands control buttons was approximately 16 inches. The two-hand control buttons were not protected against unintended operation. ABATEMENT CERTIFICATION REQUIRED
