29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    This violation recently occurred on June 11, 2012, at 2227 Waterrock, Allen TX; where employees were decking a two story single family home. Scrap boards with protruding nails were not kept clear of work areas.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    This violation recently occurred on June 11, 2012, at 2227 Waterrock Dr. in Allen, TX; where employees were decking a two story single family residence without wearing hard hats.     CAMPOS CORNICE AND DECKING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.100(a), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 309548428, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON MARCH 20, 2008, WITH RESPECT TO A WORKPLACE LOCATED AT 1914 ESPARANZA, ALLEN, TX.

29 CFR 1926.304(d): Guarding. All portable, power-driven circular saws were not equipped with guards above and below the base plate or shoe. The upper guard did not cover the saw to the depth of the teeth, except for the minimum arc required to permit the base to be tilted for bevel cuts. The lower guard did not cover the saw to the depth of the teeth, except for the minimum arc required to allow proper retraction and contact with the work. When the tool is withdrawn from the work, the lower guard did not automatically and instantly return to the covering position:    This violation recently occurred on June 11, 2012, at 2227 Waterrock Dr., in Allen, TX; where employees were decking a two story single family residence. Circular power saws did not have working blade guards.
