Employees used table saw that was not plugged into a GFCI outlet. Observed on or about 09 July 2012.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, was not used as necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of the eyes through physical contact. a) 200 Central Avenue, White Plains, New York. Employees working with carpentry tools were not wearing eye protection in the form of safety glasses with side sheilds. Observed on or about 09 July 2012.
