29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, moving parts, flying chips and sparks: (a) Facility, Building 18, Avenue C, Leetsdale, PA: On or about 06/28/12, machine guarding was not provided on the Branson AES welder to protect the operator from the hazard of the moving parts of the sleeve arm during machine operation. There was no guard in place to reduce the gap between the frame and sleeve arm. An employee suffered an amputation of their left ring finger up to the first knuckle when the employee's finger became caught between the machine frame and moving arm during the return operation of the arm to the starting position.
