29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: (a) Jobsite, North St. Clair Street, Pittsburgh, PA: On or about 06/29/12, there was no competent person on site to conduct daily inspections of the excavation and adjacent areas. No protective systems were in use to protect employees from potential cave-ins and evidence of fissures in the vertical walls was observed. In addition, a tree stood on the corner of the excavation and had its root system cut which presented the potential for collapse as well.
