The employer did not ensure that a ground fault circuit interrupter (GFCI) or assured equipment grounding conductor program was used to protect employees from possible electrical shocks, on or about June 29, 2012. No abatement certification or documentation required.

The employer did not ensure that regular and frequent inspections were made of the job site and equipment to identify that the extension cords were free from damage, on or about June 29, 2012.
