29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph. a) Rear roof area of the Sigma Phi Epsilon Fraternity - On or about July 9, 2012, an employee was climbing a ladder that was set on the top level of a scaffold which caused a tipping moment resulting in the employee falling 19.9' to the ground causing serious injury.
