29 CFR 1910.147(c)(7)(i):     The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:    Drainage Products, Inc., located at 100 West Main St., Haviland, OH 45851: On or about September 6, 2012, the employer failed to properly train employees to lockout/tagout Line 9 Corrugator (built by Corma, Inc., Model 620-6, S/N 7489), located in Plant 2, before employees were conducting activities such as, but not limited to, changing the set-up of the line to a different pipe size.    No additional abatement information is needed for this item.

29 CFR 1910.147(d)(2):     The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment:    Drainage Products, Inc., located at 100 West Main St., Haviland, OH 45851: On or about September 6, 2012, the employer did not ensure that Line 9 Corrugator (built by Corma, Inc., Model 620-6, S/N 7489), located in Plant 2, was properly turned off/shut down, using procedure established for the machine, when employees were conducting activities such as, but not limited to, changing the set-up of the line to a different pipe size. Employees were exposed to the unexpected startup of the machines rotating parts while conducting work that could result in caught-in and nip-point injuries.    No additional abatement information is needed for this item.

29 CFR 1910.178(o)(2):     Load(s) were being handled which exceeded the rated capacity of the industrial truck(s):    Drainage Products, Inc., located at 100 West Main St., Haviland, OH 45851: On or about September 6, 2012, the employer failed to ensure that Yale forklift # 52 (Model GDP070VXNYRE087, S/N B875V05346J) was used to handle loads not exceeding the maximum rated safe capacity of the powered industrial vehicle. The forklift with the attachment was used to transport products from Plant 3 to the yard. Employees were exposed to crushing, struck-by, caught-in-between, and tip over hazards.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Drainage Products, Inc., located at 100 West Main St., Haviland, OH 45851: On or about September 6, 2012, the employer failed to ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as rotating jumbo coilers, exposing employees to struck-by and caught-in hazards.  Employees were at risk of injury when working in immediate proximity to rotating coilers.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
