29 CFR 1910.137(b)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests in accordance with Tables I-5 and I-6: a) Drainage Products, Inc. - Haviland, Ohio: On or about November 29, 2013, the employer did not ensure that electrical protective rubber gloves used by employees were tested at least once every six months after being put into service. A maintenance employee used the gloves while accessing the inside of the operator control box (480v) to de-energize and re-energize the Corma Inc. manufactured #6 MQR Corrugator (Serial #6271).

29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of electric equipment, such as parts that were broken, bent, cut or deteriorated by corrosion, chemical action, or overheating: a) Drainage Products, Inc. - Haviland, Ohio: On or about January 10, 2014, the employer did not ensure that the operator control box on the Corma Inc. manufactured #6 MQR Corrugator (Serial #6271) was free of damaged/broken parts, in that the power disconnect switch was broken inside of the unit. This resulted in production, maintenance and supervisory personnel having to remove the protective cover to de-energize and re-energize the equipment, exposing them to electrical hazards, which could result in electrocution, electric shock, and/or arc flash/blast injuries.

29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: a) Drainage Products, Inc. - Haviland, Ohio: On or about January 10, 2014, the employer did not ensure employees were provided with training on electrical safety-related work practices, which may include topics such as, but not limited to electric shock hazards, arc flash/blast hazards, and protective equipment for the face, hands and body. Because a power disconnect switch was broken on the Corma Inc. manufactured #6 MQR Corrugator (Serial #6271), employees were required to remove the protective cover on the operator control box to de-energize and re-energize the equipment, exposing them to 480 volts of electricity that could result in electrocution, electric shock and/or arc flash/blast injuries.
