29 CFR 1926.51(f)(1): The employer did not provide adequate washing facilities for employees engaged in the application of paints, coating, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees. Such facilities shall be in near proximity to the worksite and shall be so equipped as to enable employees to remove such substances. (a) Yavapai Apache, Camp Verde, AZ, Cherry Lane Unit 5 - On or about April 27, 2012 employees were exposed to portland cement and were not provided with soap and clean water to remove harmful substances. The employees were exposed to highly caustic materials which could damage or burn the eyes and skin.

29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: (a) Yavapai Apache, Camp Verde, AZ, Cherry Lane Unit 5 - On or about April 27, 2012 employees mixing and handling wet Portland cement were not provided with appropriate chemical resistant gloves. Employees were exposed to highly caustic materials which could damage or burn the eyes and skin.
