Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a) Facility at 100 Steel Street, Aliquippa, PA: On or about July 10, 2012, employees were not protected from possible electrical hazards because the light fixture that was mounted behind the work area, had exposed wiring.
