Employee working on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about July 3, 2012, the employer did not ensure that employee(s) used fall protection during roofing operations on a steep roof. Two employee(s) were applying roofing felt on a single story residential building.
