29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.        Location: Second Floor, 9 Ives Place, Guilford, CT 06437    1. The one 5 foot 4 inch wide wall/window opening with a 14 foot 3 inch fall distance and a 26 inch high sill was not equipped with a guardail system.    2. The two 30 3/4 inch wide wall/window openings with a 14 foot 3 inch fall distance and a 26 inch high sill was not equipped with a guardrail system.    3. The one 77 inch wide wall/window opening with a 12 foot fall distance and a 26 inch high sill was not equipped with a guardrail system.    4. The two 30 3/4 inch wide wall/window openings with a 14 foot 4 inch fall distance and a 26 inch high sill was not equipped with guardrail system.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:    Location: 9 Ives Place, Guilford, CT 06437    The 24 inch wide, 13 riser stariway between the first and second floors was not equipped with at least one handrail and one stairrail system along each unprotected side.
