29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel: a.) Coastal Marine, Inc., Shop Area, Task Force 6" Bench Grinder, Model GL-150, Item 90186, 1/3hp, 3450rpm. On or about July 11, 2012, the bench grinder did not have a tongue guard installed exposing the operator to flying debris should the abrasive wheel shatter during operation.

29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders were located where cylinders could be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons: a.) Coastal Marine, Inc., Shop Area. On or about July 11, 2012, a free-standing acetylene compressed gas cylinder located next to a mobile welding cart was not placed in an assigned storage space and secured to prevent it from being knocked over or damaged when not in use.
