29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:      On 07-10-12, employees who were installing shingles on a commercial building were utilizing pneumatic nail guns without wearing eye protection.      No Abatement Certification or Abatement Documentation required.

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:      On 07-10-12, employees were working on a 4:12 pitch roof with a working surface edge greater than fourteen and one half (14.5) feet above pavement without utilizing fall protection equipment.      No Abatement Certification or Abatement Documentation required.

29 CFR 1926.503(b)(1):     The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:      On 07-10-12, employer did not provide written certification for employee fall protection safety training for five employees who did not recognized the hazard associated with working on a roof at a height greater than fourteen and one half (14.5) feet above pavement without utilizing fall protection equipment.      Abatement Certification or Abatement Documentation required.
