29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) Jobsite-1022-1024 Fairmount Ave, Philadelphia, PA- Employees were performing stucco work at approximately 18 ft. above ground level from a tubular welded scaffold that did not have a means of access such as a ladder. Observed on or about 6/15/12. Note: No abatement certification or documentation required.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: a) Jobsite-1022-1024 Fairmount Ave, Philadelphia, PA- Employees were performing stucco work at approximately 18 ft. above ground level from a tubular welded scaffold without scaffold safety training. Observed on or about 6/15/12. Note:Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement
