29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems: At the west side of the roof, on July 10, 2012, employees were placing roofing material onto the forks of the telescoping boom forklift. The employees were not protected against falls of approximatley 20 feet.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: At the west side of the roof, on July 10, 2012, employees were removing roofing material near the edge of the roof. The employees were not protected against falls of approximatley 20 feet.
