29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. (a)The employer failed to ensure that each employee conducting roofing work at 15275 Casey Circle in Brookfield, Wisconsin, were protected from falling to the lower level by the use of a fall protection system. This exposed employees to a fall hazard of approximately 16 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a)The employer did not ensure that the ladder, utilized to access the roof, was extended at least 3 feet above the roof edge. This exposed employees to a fall hazards of approximately 16 feet.
