29 CFR 1904.39(a):  Within eight (8) hours after the death of any employee from a work-related incident or the in-patient hospitalization of three or more employees as a result of a work-related incident, the employer did not orally report the fatality/multiple hospitalization by telephone or in person to the Area Office of the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor, that is nearest to the site of the incident:    a. On or about July 10, 2012 the employer did not timely report a fatality of an employee involved in a work-related accident within 8 hours. An employee was run over by a 30 foot Rolls Rite trailer that was being pulled by a 673 John Deere tractor at approximately 7:09pm. The employee died at about 7:49pm on July 9, 2012. The employer did not report the fatality to OSHA.    ABATEMENT CERTIFICATION AND DOCUMENTATION ARE NOT REQUIRED.
