29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles were connected or altered in a manner which prevented proper continuity of the electric equipment grounding conductor at the point where plugs were attached to receptacles: Dietary: The plug provided on the Powers Chest Cooler used to store milk could not be properly plugged into the wall receptacle because the plug assembly needed to be plugged into the lower receptacle due to the cord configuration. The cooler was energized through the top receptacle, above another plug, which did not allow for proper contact of the plug's ground prong in the receptacle and could have caused excessive heat from increased resistance from not being plugged in all the way.
