29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift:    a)  American Builders on or about July 10 2012, at the address of 201 Lytle St. Fostoria, OH: The employer failed to ensure employees working from two different aerial lifts were adequately protected from falls.  Employees working from the lifts were not clipped off to the basket while the platform was elevated. The employees were exposed to an approximately 11 ft fall to the ground below.
