On or about July 12, 2012, on the 4:12 pitch roof, employees conducting roofing operations were not protected from a 12 feet fall hazard by the use of a guardrail system, safety net system, or a personal fall arrest system.

On or about July 12, 2012, the 24-foot aluminum extension ladder used to access the roof had bent rungs, rails, and a defective locking device, exposing employees to a 15 feet fall hazard.
