29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:     On or about October 29, 2012, in the I&S area (Department 315), employees were using respirators, such as but not limited to, 3M half mask respirators, with facial hair, which interfered with respirator's seal and potentially presented inhalation hazards.

29 CFR 1910.134(h)(3)(iii): Self-contained breathing apparatus were not inspected monthly, maintained in a fully charged state, recharged when the pressure falls to 90% and the employer did not determine that the regulator and warning devices function properly:    On or about October 23, 2012 and times prior thereto, employees were delegated in the employer's emergency response plan to respond to a spill of chemicals such as but not limited to, Nickel Nitrate, Liquid Nitrogen, Cadmium Oxide, Cadmium Nitrate, and Sulfuric Acid, self-contained breathing apparatus's ( Scott Air-Pak Models 2.2, 3.0. & 4.5) that were supplied for the response were not maintained. Inadequate respiratory protection during a response could result in chemical anoxia and inhalation hazards.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(2) and (3) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area:     On or about October 23, 2012 in the I&S area (Dept. 315) and Department 340, the employer did not provide hazard communication training to new hires, working with hazardous chemicals, such as, but not limited to nickel powders, nickel nitrate, and sulfuric acid.  Employees were exposed to inhalation hazards and skin contact hazards.
