29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official shall be marked on plates of approved design which shall be supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate. On or about July 11, 2012 and at times prior thereto, employees entering into storage rooms with loads stored above were exposed to being crushed by the collapsing roof above.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. On or about July 11, 2012 and at times prior thereto, employees working on and around the Eagle machine were exposed to being caught by ingoing nip points and rotating parts. Machine guarding was not in place to protect employees from the hazards associated with working on or near the unguarded equipment.
