29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) shall be protected by a cover that leaves no openings more than 1 inch wide. The cover shall be securely held in place to prevent tools or materials from falling through. Flexo 320 machine: The employer exposed its employees to tripping hazards from unprotected holes which were approximately six (5) inches in width. The hole was not covered to protect employees from tripping or stepping into them which could cause injury.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a) Maintenance Shop: The milling machine that was staged in the maintenance department was not equipped with a guard, exposing employees to contact-with hazards. b) Flexo 318, slotting area: The die cutting shaft was not equipped with a guard or barrier, exposing employee to contact-with hazards.

29 CFR 1910.212(b): Anchoring fixed machinery. Machines designed for a fixed location shall be securely anchored to prevent walking or moving. Maintenance shop area: The employer exposed employees to struck-by hazards as a drill press machine was not anchored to the floor. The maintenance machinery could potentially tip-over and strike employees who were working in the area.
