Bluestem, Wichita, KS, employee(s) engaged in installing asphalt shingles were working from a 4 in 12  sloped roof without fall protection, exposing employees to a fall hazard of approximately 9 feet to the concrete below.

Bluestem, Wichita, KS, employee(s) accessed a roof using an extension ladder that did not extend three feet above the eave, exposing employees to a fall hazard of approximately 9 feet to the concrete below.
