29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: (a) Roof Deck - Employees did final connections of metal decking for the roof of the building. At the middle, the roof deck had a height of 38 feet above the ground. At the eaves, the roof deck had a height of 31 feet above the ground. The employees used a controlled decking zone for fall protection, a method of fall protection which the standard permits for heights of 30 feet or less, on or about June 6, 2012. PINNACLE STEEL OF NEW ENGLAND, INCORPORATED WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.760(a)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311586150, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON AUGUST 21, 2008, WITH RESPECT TO A WORKPLACE LOCATED AT 2 COMMERCE DRIVE, HOOKSETT, NH 03106. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
