29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) On or about July 10, 2012, Colorado Structures, Inc., as controlling/exposing employer, did not ensure the south exit door was properly locked and posted to restrict access, thus exposing employees and contractors to a fall hazard into an open trench located outside the exit door.
