29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502. a) Building site front - On or about 11 July, 2012 - Employees were installing Hardi-plank siding on the front of the new home without the use of any fall protection while working 6 feet or more above a lower level. Employees were exposed to a fall hazard of approximately 24 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Building site front - On or about 11 July, 2012 - Employees were using a ladder that did not have its side rails extended a minimum 3 feet above the upper landing surface. Employees were exposed to a fall hazard of approximately 24 feet.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. a) Building site front - On or about 11 July, 2012 - Employees were using a 50 foot extension cord with damaged insulation as a power supply to a circular saw that was being used to cut the Hardi-Plank siding. Employees were exposed to an electrical shock hazard.
