29 CFR 1910.67(c)(2)(v): A body belt was and a lanyard were not worn or attached to the boom or basket when working from an aerial lift. On June 26, 2012, and times prior to, an employee was using a aerial lift to perform building maintenance without using a body belt of lanyard. This condition exposed employee to a fall up to 25 feet to the ground below.
