OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being crushed and or struck by a falling load.    A)  At jig 1 area, on or about July 12, 2012, and at times prior to, employees operated 2 ton overhead Jib Cranes with electric hoist to lift materials.  The hook used with the hoist did not have a self closing spring operated latch across the throat of the hook to prevent material from accidentally becoming detached from the hook, exposing employees to struck-by falling material.    ABATEMENT CERTIFICATION AND DOCUMENTATIONS ARE REQUIRED.

Employees did not have lockout/tagout training, did not use lockout/tagout and the employer did not have lockout/tagout procedures, exposing employees to electrical shock and amputation hazards. 29 CFR 1910.212(a)(3)(ii):     The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    A) In the sheet metal shop, on or about July 12, 2012, and at times prior to, employees operated the Big Press Brake without a belt cover for the portion of the in-running nip point on the belt which was unguarded approximately 70 inches from the floor, exposing employees to amputation hazard.    B) In the sheet metal shop, on or about July 12, 2012, and at times prior to, employees operated Stamco Press with a guard that had a gap which was 2 inches in size running along the length of the guard, exposing employees to amputation hazard.     C) In the sheet metal shop, on or about July 12, 2012, and at times prior to, employees operated Bliss-21 with guard opening of approximately 3 inches, exposing employees to amputation hazard.    D) In the sheet metal shop, on or about July 12, 2012, and at times prior to, employees operated Bufallo Iron worker with guard space which was in excess of 2 inches and allowed employees to place their fingers in the point of operation, exposing employees to amputation hazard.    E)  In the sheet metal shop, on or about July 12, 2012, and at times prior to, employees operated Bliss Mechanical Press with guard that allowed employees to place their fingers  within 6 inches of the point of operation in two places, exposing employees to amputation hazard.    ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    A)  At jig 1 area, on or about July 12, 2012, and at times prior to, the control pendant electrical cord's outer insulation was pulled back and did not have any strain relief, the conductor was exposed and this condition exposed employees to an electrical shock hazard.    B)  At jig 1 area, on or about July 12, 2012, and at times prior to, on the electrical cord to control box # 6A, the cord's outer insulation was pulled back and did not have any strain relief, the conductor was exposed and this condition exposed employees to an electrical shock hazard.    C)  Big Press Brake, on or about July 12, 2012, and at times prior to, on the electrical cord to the on/off control, the cord's outer insulation was pulled back and did not havw any strain relief, the conductor was exposed and this condition exposed employees to an electrical shock hazard.
