29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:      On or about July 13, 2012, employees performing framing activities were using an electrical outlet box with an electrical cord connected to the temporary power source. The employees were exposed to electrical shock hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.      On or about July 13, 2012, employees performing framing activities on a residential construction project were not protected from fall hazards. Employees were exposed to unprotected falls of greater than 6 feet to the ground below.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:      On or about July 13, 2012, employees were using the top step of a stepladder as a step to gain access to the upper areas to perform framing activities. The employees were exposed to an unprotected fall of over 6 feet to the concrete below.
