29 CFR 1926.502(d)(15): Anchorages used for attachment for personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee: (a) D & M Roofing and Sheet Metal LLC at 40 Village Drive, Winter Park, Colorado 80482: On and before May 31, 2012, the employer did not ensure that each employee was protected by the use of a complete fall protection system meeting the requirements of paragraph (a)(2) of S 1926.502 of this Subpart, in that employees conducted roofing activities while on top of a three story condominium building and did not use anchorage points capable of supporting at least 5,000 pounds per employee attached. At least two employees used the eye screws that were already installed on the roofs ridge which were not capable of supporting at least 5,000 pounds per employee. Condition exposed employees to the hazard of fall from elevation at an approximate height of 25 feet or more.
